Only One Account Per Family
If you have created an account with a sibling, you must use that account to register a new sibling.
The program will give you three attempts to enter your password, then lock you out.
SPRING online registration begins on 01/19/24 and closes on 02/19/2024.
Please choose the appropriate Registration Form for HIGH or MIDDLE school.
You must hand in (or scan) your NJ State Physicals Packet (paper copy)
found in Quicklinks (top left corner of this page) to the HS school nurse olga.avagyan@pequannock.org,
MS school nurse adrianna.komeshok@pequannock.org
or AD secretary amy.tabakman@pequannock.org
Your physical must be conducted within 365 days prior to the first day of official practice in an athletic season.
Please DO NOT upload your physical paperwork to the registration site. *Scan and email to the school.
First time logging in (PLEASE READ)
Go to pequannock.org - Click on MENU - Click on Athletics - Click on Registration
Go to the bottom of the page-the sports box (Athletic Team Registration)
New User- Set up your username and password- **You will then have to wait for a confirmation email**
Go back and log in to complete the registration. You will need to fill out the information with the asterisk (*).
MAKE SURE TO HIT THE SAVE AND SUBMIT BUTTON when completed

Only sign up for the SPRING 2023-24 season
After hitting SUBMIT at the end of your registration you will receive a prompt
(You have Successfully registered) All information is saved online (do not print).

If you have Asthma or a life-threatening allergy you will need to update the MD orders with the school nurse yearly.

The NJSIAA requires our school physician to review and co-sign all physicals. Please respect the deadlines!
